It is with a heavy heart that we inform you of Carbon Health’s difficult decision to close the Diabetes / Virtual Endocrinology program. We understand this unexpected news may cause concern and uncertainty, but our focus in the coming weeks is to assist in whatever way we can to ensure a smooth transition of care.
It has been our immense pleasure, and honor, to work with you. We thank you from the bottom of our hearts for giving us the opportunity to be part of your health care team. Our hope is that your learnings through our program have empowered you to live a fulfilling life while managing diabetes and/or managing weight.
Important Details
- Our last day of clinical operations will be 11/29/23. Please schedule your follow-up visit with Dr. Wu and your diabetes educator as soon as possible so we can help you with a smooth transition in your care.
- Please be patient with message response times as we anticipate increased messages in the coming weeks.
- Please message us when you find a new endocrinologist / diabetes care team. We’ll be happy to send medical records (if outside Carbon Health) so your new care team has your latest clinical information.
FAQs
How will I be billed in the coming months till the program close date?
For all billing questions, please reach out to billilng@carbonhealth.com directly. Please be aware it may take up to 2 weeks to receive a reply.
Will I lose access to the logging features and meal rankings in the Carbon Health app?
Fortunately, the in-app logbook and meal ranking features will remain available to all Carbon Health patients, and you can continue using them for the foreseeable future.
Do note: this would be for your own self learnings and/or discussing with your Carbon primary care physician. After 11/29/23, there will be no one from your diabetes care team to review your data or troubleshoot issues.
How will I get my prescriptions filled?
If you haven’t already seen Dr. Calvin Wu recently, we encourage you to schedule a final follow-up visit with him to discuss next steps with your care. His focus will be on ensuring as smooth a transition of care as possible to another provider (or will communicate needs with your existing primary care provider), and he can refill any prescriptions you might need at that time. However, given that the next available appointment for many endocrinologists is 3-5 months away, we strongly advise you to start looking for a new endocrinologist now to avoid any gaps in your care and medications.
I would like to continue seeing Dr. Wu, Aimee, and/or Shetal - will that be possible after the virtual endocrinology program ends?
Dr. Calvin Wu will be transitioning to another role elsewhere but is actively exploring options to continue seeing patients on a part-time (1 day/week) basis. If you are interested in continuing your care with him in an exclusively virtual manner (telemedicine, with both its advantages and disadvantages), you can message us to add yourself to his list and he will contact you directly when more details are available.
Shetal Desai Rautela (Dietitian and diabetes care and education specialist), will be working elsewhere. If you are interested in continuing to see Shetal via telehealth, please send us a message stating the same. She will reach out to you with updates about her private practice and when you could start seeing her again.
At this time Aimée's availability is unclear, but do know she plans to continue providing diabetes education services.
How do I transfer my medical records?
All your medical records at Carbon will remain protected and we are happy to assist with the transfer of records to your new providers. Once you’ve found a new endocrinologist to work with please send a direct request to (f) 888-972-1912
