Step #1 – Pure Effect Challenge
- Wake with blood glucose near 200mg/dL and do not have any insulin on board other than basal insulin
- Take a Blue cartridge, do not eat, do not exercise, and wait 90 minutes
- Assess blood glucose drop
- Repeat 2–3x
- Can conduct Pure Effect Challenge at other times to see if sensitivity changes throughout the day
Step #2 – Meal Dose Effect Challenge
- Choose one meal with known amount of carbohydrate (ideally 30–50g)
- Check blood glucose prior to meal and take Blue cartridge
- Check blood glucose 90 min post meal
- Repeat 2–3x
How to Assess
- If blood glucose does not return to target (80–130mg/dL), increase to Green cartridge and repeat test 2–3x
- If have low blood glucose within 30 min of meal, then push dose to 20 min after meal
- If have low blood glucose within 45–90 min of meal, then dose too strong/need larger carb intake
Important to Keep in Mind
- Action of inhaled dose: 12 min to 1st measurable effect, 35min to peak effect, 90min return to baseline
- Full dose is in pulmonary system within 1 second of inhalation. May have cough or tickle in throat. If so, drink water.
- When to safely take correction dose—no sooner than 90 min from last inhaled dose
- Avoid combining with injected dose at all times, as it complicates evaluation of inhaled dose
- Storage:
- Unopened blister pack at room temp good for 10 days
- Opened blister pack at room temp good for 3 days
- Keep in refrigerator otherwise
- Must be out of refrigerator for 10 min prior to use
- Do not use inhaler for more than 15 days
